Office Policies

Appointment Policy

To make an appointment with one of our providers for non-urgent appointments, please contact our office during normal business hours or use one of the online methods below.

Same day appointments are available for sick visits and minor emergencies; however, these appointments usually are filled quickly. Please call early for same day appointments. These appointments are limited and are handled on a first call basis.

Please call our office or use patient portal (established patients only) at least 2 weeks in advance to schedule appointments for check-ups, well visits, medication refills, and elective in-office procedures.

If you are unable to keep your appointment, please give at least 24 hour notice so that we can make that time available for other patients.

In an effort to stay on time, please arrive promptly for your appointment. If you anticipate arriving late for your appointment, please notify us as soon as possible. It may be necessary to reschedule your appointment.

Prescription Refill Policy

If you need your current prescription refilled, please contact your pharmacy or request through patient portal.

Refill requests are processed at the end of the day. Please check with the pharmacy to see if prescription requested has been filled before calling our office.

Please do not wait until you are out of medication to request a refill.

Lab and Radiology Results

You will receive notice in the mail or through patient portal after your results are obtained and reviewed by your physician.

If you have not received results in the mail or through patient portal within 3 weeks after your test have been done, please call our office. Please do not call the office any sooner for results of your lab work or radiology results.

If there are significant abnormal results, you will be notified by phone.


Depending on your insurance plan, it may require up to 48 hours to complete a referral. Some referrals may take longer than 48 hours. Once a referral has been approved it will be faxed to specialist office and you will be notified by phone or patient portal.

Payments and Insurance

Co-payments and deductibles are due at the time of your office visit.

We must have your current insurance card and a state issued picture id or state driver's license.

If you have a question on your bill, please contact our billing service Northwest Coding, LLC at 888-238-6391 ext 2. You may also submit billing inquiries through the patient portal.


For potential life threatening emergencies, go directly to the nearest hospital or dial 911.

For established patients with other urgencies, you may call the office at 936-760-4454 and the on-call physician will be paged.
