Family Healthcare Affiliates PLLC Merger with IKP Family Medicine: Medical Records and Insurance Information
Dear Patients,
After 28 years of caring for patients and families in Montgomery County and surrounding communities, we are beginning a new chapter. We are pleased to announce that, effective January 1, 2027, Family Healthcare Affiliates PLLC will be merging with IKP Family Medicine.

Insurance and Medicare Annual Enrollment
During Medicare Annual Enrollment, you may receive a letter from HealthSpring or Humana stating that our practice is no longer in network or that you will be assigned a new Primary Care Provider (PCP). This change involves the management company associated with your coverage and does not reflect a decision by our practice to stop participating in your insurance plan. We intend to remain in network with your plan for 2027 and are working to minimize any disruption to your care.
If you receive a notice about network status or a new PCP assignment, please contact our office before changing your coverage. We can help answer questions and help you determine the appropriate next steps.
If you continue your care with us
Please complete a medical records release form so we can transfer your records to IKP Family Medicine. You can find the Medical Records Release Form at our office or on our website at Medical Records Release to IKP Family Medicine Form. See the Instructions for Medical Record Release Form section below.
If you have an HMO or Medicare Advantage plan, including HealthSpring, Aetna Medicare Advantage, Humana HMO or PPO Medicare Advantage, or BCBS, contact your insurance plan directly to confirm network participation and any required PCP change. If you stay with our practice, you will likely need to contact your carrier after January 1, 2027, to select us as your PCP. Ask your plan when they can process the change and what effective date will apply.
If you choose a different provider
Please complete a medical records release form and include the email address of the new practice where you would like your records sent. If your plan requires a designated PCP, contact your insurance company to arrange the change for January 1, 2027, and confirm its effective date. Medical Records Release Form to Another Provider. You can complete the form electronically or by hand, but you must print and sign it.
Here are several ways to return your completed Medical Records Release Form:
1. Upload to your Patient Portal. Attach it to a message addressed to Family Healthcare Affiliates PLLC.
2. Fax to the number at the top of the form. Make it to the attention of KUrban.
3. Email to KUrban@fhaconroe.net.
4. Drop off at the office's front desk.
